Description: Form DD 1172-2 is required for contractors to receive a Common Access Card (CAC). Other information entered in this section include overseas assignment begin and end date, eligibility effective and expiration date, sponsors official name which should include last, first and middle initials, unit organization, title, pay grade ,signature and date verified.

1. A travel authorization produced by the sponsoring DoD Component, indicating an assignment outside the United States. A death certificate, if the child dies. If the sponsor adopts his or her step-children, they remain covered as adopted children even if the marriage ends in divorce.

If you are a sponsor, and you want to enroll your dependents in DEERS, you have to fill out sections I, II and V. If you are an eligible employee applying for ID card, you have to fill out sections I and II. Personnel of Department of Defence (DoD) have to complete section III.

The following instructions will offer some guidelines when filling the DD Form 1172-2. 2. A common law marriage certificate, a court order, or a written SJA opinion that a common law marriage is recognized in the relevant State or U.S. jurisdiction, is also accepted.

DD form 1172-2 is one of the forms that are filled in order for one to acquire a Uniformed Services Identification card (USID). Marriage certificate (Note 1) or Death certificate (Note 4) or Change in financial status (Note 7) or Medical sufficiency statement (Note 9).

If the marriage ends in divorce, the step-children lose eligibility on the date the divorce decree is final. DD Form 214 (Note 3) or Military order (Note 4) or Commissioning oath (Note 4) or Enlistment contract (Note 4). The new DD Form 1172-2 will require obtaining personal information from both the government employee and dependent.
